一.工作開展情況

嚴格執行新農合的藥(yao)品目錄合理規(gui)范用藥(yao)。堅持以病人為中心的服務準則。

使用(yong)新(xin)(xin)農合(he)專用(yong)處方并認真填寫《新(xin)(xin)農合(he)醫療證》和門診(zhen)登記,參合(he)農民就診(zhen)時確認身份后。嚴格控制開大處方,不超標收費(fei),補償賬本上(shang)親自簽字及按(an)手印,以(yi)防冒領資(zi)金。

過(guo)期(qi)藥(yao)品(pin)及劣(lie)質(zhi)藥(yao)品(pin),藥(yao)品(pin)上嚴禁假藥(yao)。藥(yao)品(pin)必(bi)須經過(guo)正規渠道進取。

在分析(xi)取得成績(ji)的同時,我們也應該清(qing)醒地(di)看到,當(dang)前我縣新(xin)型農村(cun)合作(zuo)醫(yi)療(liao)工作(zuo)與農民群(qun)眾的期望(wang)和(he)上級部門(men)的要求相比還(huan)存(cun)在一定的差距(ju):一是(shi)籌(chou)資工作(zuo)難度較大,農民參合率仍然不高(gao)。二是(shi)合作(zuo)醫(yi)療(liao)制度還(huan)不夠完善,工作(zuo)的規范化程度還(huan)不高(gao)。三(san)是(shi)合作(zuo)醫(yi)療(liao)保障水平還(huan)不高(gao),與群(qun)眾的要求還(huan)有差距(ju)。四是(shi)醫(yi)療(liao)機(ji)(ji)構的服務能力(li)和(he)水平還(huan)需(xu)進一步(bu)提(ti)高(gao)。針對(dui)這些(xie)問題,我們要切實(shi)增強(qiang)工作(zuo)的責任感(gan)和(he)使命感(gan),認(ren)真調(diao)查研究,強(qiang)化工作(zuo)措施,健(jian)全籌(chou)資機(ji)(ji)制、運行機(ji)(ji)制和(he)監督機(ji)(ji)制,促進全縣新(xin)型農村(cun)合作(zuo)醫(yi)療(liao)工作(zuo)健(jian)康發展。

改進少數民族新農合狀況

一、引言

二、研(yan)究設計與數字(zi)處(chu)理(li)

為(wei)了(le)從(cong)多個(ge)角度了(le)解(jie)新(xin)農(nong)合在少(shao)(shao)數民聚居(ju)地(di)(di)區運行(xing)的(de)(de)實(shi)際(ji)狀況,客觀(guan)評價和(he)判斷新(xin)農(nong)合運行(xing)幾年中(zhong)實(shi)際(ji)存在的(de)(de)問題(ti),尋(xun)找(zhao)進一步鞏固發展的(de)(de)可行(xing)性(xing)(xing)思路(lu),使(shi)惠及(ji)世界人口最(zui)龐大的(de)(de)中(zhong)國新(xin)農(nong)合制度在少(shao)(shao)數民族(zu)聚居(ju)地(di)(di)區不(bu)斷趨于成(cheng)熟和(he)完善,并能結合不(bu)同地(di)(di)區實(shi)際(ji),充分(fen)考慮民族(zu)性(xing)(xing)和(he)地(di)(di)域性(xing)(xing)特(te)點,建立富(fu)有中(zhong)國民族(zu)特(te)色的(de)(de)新(xin)農(nong)合,課題(ti)組選擇(ze)了(le)內蒙古與云南兩個(ge)典型的(de)(de)少(shao)(shao)數民族(zu)聚居(ju)省(sheng)份的(de)(de)部分(fen)旗縣(xian)進行(xing)了(le)問卷(juan)調查。

新農合工作計劃

1、加強和完善新(xin)農合制(zhi)度(du)建(jian)設(she)并(bing)完善信(xin)息

認真貫徹落實上級文件(jian)精神,嚴格執行省、市、州新(xin)(xin)農(nong)合(he)文件(jian)精神,結合(he)我縣實際(ji),制定(ding)并下(xia)發《縣2016年新(xin)(xin)型(xing)農(nong)村合(he)作(zuo)醫療制度實施方(fang)案》,完善新(xin)(xin)農(nong)合(he)相關配套政策和規定(ding),通過(guo)相關配套制定(ding)的(de)建(jian)立,構筑新(xin)(xin)型(xing)農(nong)村合(he)作(zuo)醫療基金收支平穩(wen)運行的(de)保(bao)障機(ji)制。及時(shi)完成參(can)合(he)人員信(xin)息(xi)核實、錄(lu)入工作(zuo),對參(can)合(he)錯(cuo)誤信(xin)息(xi)給(gei)予糾正。

2、進(jin)一步(bu)推進(jin)分級診療工作

按《關于(yu)進(jin)一步完(wan)善新農(nong)合補償政策推進(jin)分(fen)級(ji)診療工作(zuo)的通(tong)知》文件要求,為(wei)切(qie)實加強(qiang)新合資金管理,保障基(ji)金安全,提高基(ji)金使用效率,充分(fen)發揮新農(nong)合制(zhi)度杠桿作(zuo)用,我辦支持(chi)建立(li)和完(wan)善分(fen)級(ji)診療制(zhi)度。

3、進(jin)一步(bu)加大宣傳力度

制(zhi)作(zuo)并發放2016年新農合制(zhi)度的宣傳(chuan)(chuan)(chuan)資(zi)料,把宣傳(chuan)(chuan)(chuan)工(gong)(gong)作(zuo)作(zuo)為(wei)一項長期(qi)任務抓(zhua)緊、抓(zhua)好、抓(zhua)實(shi),用典(dian)型事例現身(shen)說(shuo)法,使(shi)(shi)參合患者(zhe)感受(shou)到實(shi)實(shi)在(zai)在(zai)的好處。結合受(shou)益群眾(zhong)典(dian)型事例,利(li)用廣播、電視、宣傳(chuan)(chuan)(chuan)欄(lan)、宣傳(chuan)(chuan)(chuan)手冊(ce)等宣傳(chuan)(chuan)(chuan)媒體和宣傳(chuan)(chuan)(chuan)工(gong)(gong)具,廣泛宣傳(chuan)(chuan)(chuan),大造輿論,使(shi)(shi)農民群眾(zhong)充分了解參加合作(zuo)醫療后(hou)自己的權益、義務,明白看病報銷辦(ban)法、程(cheng)序和比例,消除農民疑慮和擔心,增強他們互助共濟的參合意(yi)識。

自11月1日(ri)正(zheng)式實施,縣(xian)(xian)(xian)是省新型農村合作醫療第一(yi)批試點(dian)縣(xian)(xian)(xian)之一(yi)。已運(yun)行近五年時間。經過幾年的探(tan)索,縣(xian)(xian)(xian)初步(bu)建立了一(yi)套行之有效的管理體制和運(yun)行機制,補償政策逐(zhu)(zhu)步(bu)完善(shan),補償水(shui)平(ping)逐(zhu)(zhu)步(bu)提高,報銷程序(xu)逐(zhu)(zhu)步(bu)簡化,監管能力逐(zhu)(zhu)步(bu)加(jia)強(qiang),資金(jin)使用合理安全,農民(min)信任度明顯提高,農民(min)參合積(ji)極(ji)

其中統(tong)(tong)籌(chou)資(zi)(zi)(zi)金(jin)支(zhi)出(chu)(chu)(chu)(chu)9029萬(wan)(wan)元(yuan),性不斷提(ti)高。全(quan)縣新(xin)農合(he)參(can)合(he)率(lv)由的(de)81%上升至的(de)95%。-全(quan)縣新(xin)農合(he)資(zi)(zi)(zi)金(jin)支(zhi)出(chu)(chu)(chu)(chu)9751萬(wan)(wan)元(yuan)。補(bu)償(chang)186818人(ren)次。統(tong)(tong)籌(chou)資(zi)(zi)(zi)金(jin)支(zhi)出(chu)(chu)(chu)(chu)包(bao)括住院補(bu)償(chang)7836萬(wan)(wan)元(yuan),住院分娩(mian)補(bu)償(chang)551萬(wan)(wan)元(yuan),慢性病補(bu)償(chang)154萬(wan)(wan)元(yuan),村(cun)衛(wei)生站觀察治療(liao)(門(men)診(zhen)統(tong)(tong)籌(chou))補(bu)償(chang)488萬(wan)(wan)元(yuan)。統(tong)(tong)籌(chou)資(zi)(zi)(zi)金(jin)使(shi)用率(lv)平均(jun)為90%,受益率(lv)平均(jun)為7%,補(bu)償(chang)比平均(jun)為30%。縣共有(you)690257位農民(min)參(can)加新(xin)農合(he),1-6月,新(xin)農合(he)資(zi)(zi)(zi)金(jin)支(zhi)出(chu)(chu)(chu)(chu)2605萬(wan)(wan)元(yuan),其中統(tong)(tong)籌(chou)資(zi)(zi)(zi)金(jin)支(zhi)出(chu)(chu)(chu)(chu)2364萬(wan)(wan)元(yuan),補(bu)償(chang)35400人(ren)次,家庭賬(zhang)戶(hu)支(zhi)出(chu)(chu)(chu)(chu)241萬(wan)(wan)元(yuan)。新(xin)農合(he)的(de)實施,有(you)效(xiao)地減輕了(le)農民(min)的(de)醫療(liao)費用負擔,緩(huan)解了(le)廣大農民(min)因病致貧(pin)、因病返貧(pin)問題,為促進農村(cun)經濟發展、維護社會(hui)穩定發揮了(le)積極作用。縣榮獲“全(quan)省(sheng)新(xin)型(xing)農村(cun)合(he)作醫療(liao)試點先進縣”稱號。

新農合制度研究論文

一、欠發達地區新型農村合作醫(yi)療的環(huan)境特(te)點

1.對(dui)推行(xing)和(he)(he)參加“新(xin)農合(he)(he)”的(de)認識(shi)(shi)不(bu)(bu)(bu)(bu)到(dao)位(wei)。在欠發(fa)達地(di)區(qu),推行(xing)新(xin)農合(he)(he)制(zhi)度首先遇(yu)到(dao)的(de)是(shi)觀(guan)念(nian)轉變問題。一(yi)(yi)(yi)方(fang)(fang)面(mian)由于歷史、經濟、觀(guan)念(nian)等(deng)多方(fang)(fang)面(mian)的(de)情(qing)況,地(di)方(fang)(fang)政(zheng)府對(dui)建立和(he)(he)完善“新(xin)農合(he)(he)”制(zhi)度的(de)長期性、艱(jian)巨(ju)性和(he)(he)復雜性認識(shi)(shi)不(bu)(bu)(bu)(bu)足,對(dui)有關政(zheng)策(ce)理解不(bu)(bu)(bu)(bu)透,工作不(bu)(bu)(bu)(bu)細,宣傳發(fa)動不(bu)(bu)(bu)(bu)深入,管理粗放,使有些(xie)政(zheng)策(ce)措施未能很好(hao)地(di)貫徹落實(shi);另一(yi)(yi)(yi)方(fang)(fang)面(mian),一(yi)(yi)(yi)些(xie)農民對(dui)“新(xin)農合(he)(he)”醫(yi)療(liao)制(zhi)度還是(shi)存有疑慮:擔心政(zheng)府的(de)資金不(bu)(bu)(bu)(bu)能落實(shi)到(dao)位(wei),擔心醫(yi)療(liao)費用能不(bu)(bu)(bu)(bu)能合(he)(he)理、及時(shi)、方(fang)(fang)便得到(dao)報銷,擔心醫(yi)療(liao)費用過高、服務(wu)不(bu)(bu)(bu)(bu)好(hao)、得不(bu)(bu)(bu)(bu)到(dao)實(shi)惠等(deng)等(deng)。

2.醫(yi)療(liao)資(zi)(zi)源缺乏(fa)和(he)政府監(jian)管(guan)缺位(wei)。醫(yi)療(liao)衛(wei)生資(zi)(zi)源是人(ren)民健康保障(zhang)的前提和(he)基礎,而(er)對(dui)于經(jing)濟(ji)社(she)會相對(dui)落后的中西部省份(fen)來說,農村新(xin)型合作醫(yi)療(liao)工作首先要面對(dui)的現實問題就是衛(wei)生資(zi)(zi)源總量缺乏(fa)和(he)人(ren)均衛(wei)生資(zi)(zi)源的嚴(yan)重不(bu)足。同時有(you)效(xiao)監(jian)管(guan)這些資(zi)(zi)源,促使(shi)農村醫(yi)療(liao)機構提高(gao)水平、保證(zheng)質量、規(gui)范(fan)行為、控制費(fei)用(yong)就成了(le)合作醫(yi)療(liao)制度有(you)效(xiao)運(yun)行的關(guan)鍵,但是現實情況是,越是欠發達地(di)區,監(jian)管(guan)越是不(bu)利。

3.農村(cun)居(ju)民(min)收入水平低(di)。部分貧困的農村(cun)居(ju)民(min)受到(dao)(dao)收入水平低(di)的影響(xiang)而沒(mei)有參與新型(xing)(xing)合(he)(he)作醫(yi)療(liao),也(ye)影響(xiang)到(dao)(dao)新型(xing)(xing)合(he)(he)作醫(yi)療(liao)的資金(jin)籌集,最終影響(xiang)到(dao)(dao)新型(xing)(xing)合(he)(he)作醫(yi)療(liao)的參與率。

4.城(cheng)市化水(shui)(shui)平低。農村(cun)人(ren)(ren)口比例高于全國平均(jun)水(shui)(shui)平,加上人(ren)(ren)均(jun)收(shou)入(ru)水(shui)(shui)平低,農村(cun)貧(pin)(pin)困人(ren)(ren)口和接近(jin)于貧(pin)(pin)困線(xian)的人(ren)(ren)口多(duo)。在實(shi)行新型合(he)作醫療的過程中,要(yao)(yao)保(bao)證把(ba)低收(shou)入(ru)家(jia)庭納入(ru)進來,必然(ran)要(yao)(yao)幫助他們解決籌資問題,同時還要(yao)(yao)相應加大醫療救(jiu)助的力(li)度。

5.地方財(cai)(cai)(cai)(cai)政壓力大。以(yi)經濟(ji)欠(qian)發達(da)的(de)(de)江(jiang)西(xi)省(sheng)為(wei)例,2005年,江(jiang)西(xi)省(sheng)年末總人(ren)(ren)口4311萬人(ren)(ren),地方財(cai)(cai)(cai)(cai)政支出564億元,地方財(cai)(cai)(cai)(cai)政收入252.9億元。與(yu)東部地區人(ren)(ren)口基本接近的(de)(de)遼寧省(sheng)相比,2005年地方財(cai)(cai)(cai)(cai)政收入674億元,地方財(cai)(cai)(cai)(cai)政支出1182億元,可見差距十(shi)分(fen)明顯(xian)。這樣,合作(zuo)醫療管理(li)部門運轉經費嚴重(zhong)短缺,而這些地區的(de)(de)政府財(cai)(cai)(cai)(cai)力又非常有限(xian),很難自行解(jie)決。在市、縣級(ji)越是經濟(ji)落后的(de)(de)地區必(bi)然影(ying)響(xiang)到合作(zuo)醫療管理(li)工作(zuo)的(de)(de)開展。即使按照現有的(de)(de)標準,由于(yu)合作(zuo)醫療覆蓋面(mian)的(de)(de)擴(kuo)大和參(can)合農(nong)民人(ren)(ren)數的(de)(de)不斷(duan)增加,省(sheng)級(ji)財(cai)(cai)(cai)(cai)政的(de)(de)補助資金(jin)總數將大幅度增加,將對地方財(cai)(cai)(cai)(cai)政構成(cheng)較(jiao)大壓力。
