中醫學專業臨床實習質量監控體系探討

摘要:中醫學專業本科學生臨床實習能力水平的高低是評價醫學教育質量的重要指標之一,如何提高中醫學專業學生臨床實習質量,確保實踐型、創新型、創業型的綜合性中醫人才的培養效果,需要建立起完整、有效的臨床實習質量監控體系,實現(xian)對(dui)中醫學(xue)專業臨床(chuang)實習質量的(de)有(you)效監控(kong)。作為高等中醫藥(yao)院(yuan)校,多年(nian)來(lai)筆者學(xue)校在(zai)長(chang)期的(de)臨床(chuang)實習管理過程中,不斷采取各種舉措大力加強學(xue)生(sheng)臨床(chuang)實習工作,積極創造條(tiao)件,采取有(you)力措施將各項工作落到實處,深入、全面地(di)做(zuo)好臨床(chuang)實習質量監控(kong)工作。

中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)是實(shi)踐(jian)性學(xue)(xue)科(ke),決(jue)定了(le)中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)人(ren)(ren)才培(pei)(pei)養必須(xu)以臨床(chuang)(chuang)(chuang)實(shi)踐(jian)訓練為(wei)核心,臨床(chuang)(chuang)(chuang)實(shi)習(xi)作為(wei)中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)人(ren)(ren)才培(pei)(pei)養的重要(yao)教學(xue)(xue)環節,是中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)專(zhuan)(zhuan)業(ye)學(xue)(xue)生(sheng)綜合運(yun)用基礎理論知識和專(zhuan)(zhuan)業(ye)技(ji)能的重要(yao)階(jie)段,其通過在臨床(chuang)(chuang)(chuang)實(shi)踐(jian)中(zhong)(zhong)不(bu)斷提升知識運(yun)用能力(li)(li)、動手能力(li)(li)、獨(du)立分析問題(ti)和解(jie)決(jue)問題(ti)的能力(li)(li),實(shi)現從(cong)醫(yi)(yi)(yi)學(xue)(xue)生(sheng)向臨床(chuang)(chuang)(chuang)醫(yi)(yi)(yi)師角色(se)的重要(yao)轉換。如何提高(gao)中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)專(zhuan)(zhuan)業(ye)學(xue)(xue)生(sheng)臨床(chuang)(chuang)(chuang)實(shi)習(xi)質(zhi)量(liang),確保實(shi)踐(jian)型、創(chuang)新型、創(chuang)業(ye)型的綜合性中(zhong)(zhong)醫(yi)(yi)(yi)人(ren)(ren)才的培(pei)(pei)養效(xiao)果,一直以來(lai)都是各中(zhong)(zhong)醫(yi)(yi)(yi)藥院(yuan)校積極思考和探索的問題(ti),要(yao)解(jie)決(jue)中(zhong)(zhong)醫(yi)(yi)(yi)藥人(ren)(ren)才培(pei)(pei)養質(zhi)量(liang)問題(ti),需要(yao)建(jian)立起完整、有(you)(you)效(xiao)的臨床(chuang)(chuang)(chuang)實(shi)習(xi)質(zhi)量(liang)監控(kong)體(ti)系,實(shi)現對(dui)中(zhong)(zhong)醫(yi)(yi)(yi)學(xue)(xue)專(zhuan)(zhuan)業(ye)臨床(chuang)(chuang)(chuang)實(shi)習(xi)質(zhi)量(liang)的有(you)(you)效(xiao)監控(kong)。

1臨床實習質量監控體系研究背景

高等醫學教育長期以來都十分重視臨床實習,確保臨床實習“教學質量”成為各高校的關注重點,并開展了促進臨床實習教學質量提高和監控的各式研究。川北醫學院在長期的質量監控研究中把科學合理的質量標準作為實施質量控制的前提,認為沒有質量標準的監控是無目的的監控[1]。浙江醫學高等專科學校引進把以人為本、以過程為本、以質量為本作為管理理念的ISO9000質量管理體系,通過過程管理,監控并反饋實習效果,提高實習質量[2]。安徽新華學院藥學院從臨床實習文件、實習流程、實習監管和實習考核4個環節對畢業實習質量監控體系進行了初步分析研究[3]。湖北中醫藥大學藥學院張怡等老師從管理模式、信息反饋方式、畢業實習質量評價、制度保障這4個方面對如何構建畢業實習質量監控和信息反饋體系進行了探討,認為(wei)(wei)畢(bi)(bi)(bi)業實習(xi)(xi)(xi)(xi)質(zhi)(zhi)量監(jian)控和(he)(he)信(xin)息(xi)反饋體系(xi)(xi)的(de)(de)成功構建將打破傳統教(jiao)(jiao)學(xue)管理模(mo)式,加快學(xue)校(xiao)的(de)(de)教(jiao)(jiao)學(xue)管理信(xin)息(xi)化建設(she),提高教(jiao)(jiao)學(xue)管理的(de)(de)質(zhi)(zhi)量和(he)(he)效能[4]。南京(jing)醫科大(da)學(xue)就把“臨床(chuang)提升”作為(wei)(wei)發展戰略,注重(zhong)校(xiao)院協(xie)同,完善(shan)醫學(xue)生(sheng)臨床(chuang)實踐能力培養體系(xi)(xi),并(bing)認為(wei)(wei)對臨床(chuang)教(jiao)(jiao)學(xue)的(de)(de)檢查是落實教(jiao)(jiao)學(xue)質(zhi)(zhi)量監(jian)控的(de)(de)必(bi)要手(shou)段[5]。第(di)三(san)(san)軍醫大(da)研究表(biao)明,對臨床(chuang)實習(xi)(xi)(xi)(xi)質(zhi)(zhi)量效果(guo)的(de)(de)評價,宜從實習(xi)(xi)(xi)(xi)條件、實習(xi)(xi)(xi)(xi)過(guo)程和(he)(he)實習(xi)(xi)(xi)(xi)效果(guo)三(san)(san)方(fang)面進行,并(bing)以其(qi)中的(de)(de)臨床(chuang)實習(xi)(xi)(xi)(xi)效果(guo)作為(wei)(wei)重(zhong)點[6]。廣州醫學(xue)院通過(guo)由(you)學(xue)院組織的(de)(de)畢(bi)(bi)(bi)業考試加強了對畢(bi)(bi)(bi)業實習(xi)(xi)(xi)(xi)質(zhi)(zhi)量的(de)(de)全面監(jian)控[7]。江(jiang)西中醫藥(yao)大(da)學(xue)認為(wei)(wei)臨床(chuang)實習(xi)(xi)(xi)(xi)考核是檢驗(yan)臨床(chuang)實習(xi)(xi)(xi)(xi)效果(guo)的(de)(de)重(zhong)要途徑之一(yi),對發現(xian)臨床(chuang)實習(xi)(xi)(xi)(xi)薄弱環(huan)節(jie)、提高臨床(chuang)實習(xi)(xi)(xi)(xi)質(zhi)(zhi)量具有重(zhong)要意(yi)義(yi)[8]。

2中醫學專業臨床實習質量監控體系的建立

中(zhong)(zhong)醫(yi)學專(zhuan)業(ye)本(ben)科學生臨(lin)(lin)床實(shi)習(xi)(xi)(xi)能力(li)水(shui)平的(de)(de)高(gao)低(di)是(shi)評價中(zhong)(zhong)醫(yi)藥(yao)教(jiao)(jiao)育質量的(de)(de)重要(yao)指標之一。中(zhong)(zhong)醫(yi)臨(lin)(lin)床實(shi)踐(jian)教(jiao)(jiao)學作(zuo)為高(gao)等(deng)中(zhong)(zhong)醫(yi)教(jiao)(jiao)學的(de)(de)重要(yao)組成部分,對培養學生的(de)(de)實(shi)踐(jian)動手能力(li)、知(zhi)識運用和創新能力(li)、中(zhong)(zhong)醫(yi)臨(lin)(lin)床思(si)維能力(li)、適(shi)應社會能力(li),具有極其重要(yao)的(de)(de)意義[9]。作(zuo)為高(gao)等(deng)中(zhong)(zhong)醫(yi)藥(yao)院校,多年來福(fu)建中(zhong)(zhong)醫(yi)藥(yao)大學在長期的(de)(de)臨(lin)(lin)床實(shi)習(xi)(xi)(xi)管理過(guo)程中(zhong)(zhong),積(ji)極探(tan)索臨(lin)(lin)床教(jiao)(jiao)學管理方法,采取各種舉措大力(li)加強(qiang)學生臨(lin)(lin)床實(shi)習(xi)(xi)(xi)教(jiao)(jiao)學管理工作(zuo),強(qiang)烈(lie)感(gan)受(shou)到建立起中(zhong)(zhong)醫(yi)學專(zhuan)業(ye)臨(lin)(lin)床實(shi)習(xi)(xi)(xi)質量監控體系,是(shi)不(bu)斷提高(gao)臨(lin)(lin)床實(shi)習(xi)(xi)(xi)質量的(de)(de)關鍵(jian),是(shi)培養出合格的(de)(de)中(zhong)(zhong)醫(yi)臨(lin)(lin)床醫(yi)生的(de)(de)必然要(yao)求。

2.1建立臨床實習質(zhi)量監控管理(li)機(ji)制

2.4加強基地檢查(cha)和評審,全面提升臨床教學工作

臨(lin)(lin)床(chuang)(chuang)(chuang)實(shi)(shi)習質(zhi)量保障體(ti)系涉及優(you)化(hua)基地(di)建設(she)(she)與(yu)(yu)管(guan)理、完善(shan)質(zhi)量監控與(yu)(yu)評(ping)價系統[10]。因教(jiao)(jiao)(jiao)學(xue)基地(di)等(deng)級、師資(zi)(zi)水平、管(guan)理模式(shi)(shi)等(deng)不同,存在著教(jiao)(jiao)(jiao)學(xue)模式(shi)(shi)、帶(dai)教(jiao)(jiao)(jiao)方(fang)式(shi)(shi)、考(kao)試模式(shi)(shi)等(deng)的差異(yi),導致各臨(lin)(lin)床(chuang)(chuang)(chuang)基地(di)的教(jiao)(jiao)(jiao)學(xue)質(zhi)量參差不齊[11]。由學(xue)校領(ling)導帶(dai)隊(dui),臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)專家、行(xing)政管(guan)理人員(yuan)等(deng)共同組(zu)成教(jiao)(jiao)(jiao)學(xue)檢(jian)(jian)查(cha)團隊(dui),以定(ding)期檢(jian)(jian)查(cha)與(yu)(yu)非定(ding)期巡視相結合(he)的方(fang)式(shi)(shi),按教(jiao)(jiao)(jiao)學(xue)基地(di)建設(she)(she)計劃,依據臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)基地(di)評(ping)估(gu)(gu)指標(biao)體(ti)系和專業檢(jian)(jian)查(cha)項(xiang)目(mu)標(biao)準(zhun),通過(guo)查(cha)閱(yue)資(zi)(zi)料、實(shi)(shi)地(di)走(zou)訪(fang)、師生座談、現(xian)場考(kao)核等(deng)形式(shi)(shi)從醫(yi)院教(jiao)(jiao)(jiao)學(xue)基本(ben)條件(jian)、臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)管(guan)理、教(jiao)(jiao)(jiao)學(xue)實(shi)(shi)施情況(kuang)、實(shi)(shi)習生管(guan)理、臨(lin)(lin)床(chuang)(chuang)(chuang)帶(dai)教(jiao)(jiao)(jiao)能力及師資(zi)(zi)管(guan)理等(deng)方(fang)面(mian),對(dui)(dui)醫(yi)院的臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)各個環節(jie)進行(xing)綜(zong)合(he)考(kao)察(cha)和判斷,對(dui)(dui)教(jiao)(jiao)(jiao)學(xue)基地(di)建設(she)(she)進行(xing)監督、檢(jian)(jian)查(cha)和評(ping)估(gu)(gu),結果及時反(fan)饋到(dao)教(jiao)(jiao)(jiao)學(xue)基地(di),建立競爭、淘汰機(ji)制(zhi)和評(ping)估(gu)(gu)表彰(zhang)獎勵制(zhi)度,以提高臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)基地(di)自身建設(she)(she)的自覺性、自律性,從全(quan)局(ju)上改進教(jiao)(jiao)(jiao)學(xue)工作,推(tui)動臨(lin)(lin)床(chuang)(chuang)(chuang)教(jiao)(jiao)(jiao)學(xue)工作水平的全(quan)面(mian)提高。

3結語

中(zhong)醫(yi)學(xue)專業(ye)臨(lin)(lin)(lin)床(chuang)(chuang)實習(xi)質量(liang)(liang)監控體系(xi)的研究(jiu)是一項(xiang)系(xi)統工程(cheng),在(zai)醫(yi)教(jiao)(jiao)協同背景下深(shen)化(hua)醫(yi)學(xue)人才培養改(gai)革,該校(xiao)以臨(lin)(lin)(lin)床(chuang)(chuang)教(jiao)(jiao)學(xue)基地建(jian)設為抓(zhua)手(shou),加大經費投(tou)入(ru),激勵基地教(jiao)(jiao)學(xue)投(tou)入(ru)熱情,在(zai)改(gai)善教(jiao)(jiao)學(xue)條(tiao)件、加強臨(lin)(lin)(lin)床(chuang)(chuang)技能(neng)實訓中(zhong)心建(jian)設、改(gai)革人才培養模式、提(ti)升教(jiao)(jiao)學(xue)能(neng)力(li)等(deng)方面(mian)提(ti)供支(zhi)持。健全學(xue)校(xiao)與醫(yi)院協調機(ji)制(zhi),密切學(xue)校(xiao)與醫(yi)院合作,強化(hua)醫(yi)德素養、中(zhong)醫(yi)藥思維、科學(xue)思維和(he)臨(lin)(lin)(lin)床(chuang)(chuang)實踐能(neng)力(li)培養。樹立“以教(jiao)(jiao)學(xue)為中(zhong)心、以醫(yi)療(liao)為基礎、以科研為先導”的指導思想,實現教(jiao)(jiao)學(xue)、醫(yi)療(liao)、科研三者的有機(ji)結合,積(ji)極創造條(tiao)件,采取有力(li)措施將各項(xiang)工作落到實處,只有這樣才能(neng)深(shen)入(ru)、全面(mian)地做(zuo)好臨(lin)(lin)(lin)床(chuang)(chuang)實習(xi)質量(liang)(liang)監控工作,才能(neng)充分保證和(he)逐步(bu)提(ti)高(gao)(gao)高(gao)(gao)校(xiao)中(zhong)醫(yi)學(xue)專業(ye)臨(lin)(lin)(lin)床(chuang)(chuang)實習(xi)質量(liang)(liang)。
